Medical Records and Prior Reports
With certain surgical and imaging procedures, we require patients bring prior films and reports with them on the date of their service. This prior information assists our physicians in diagnosis and treatment planning. If you need assistance obtaining your prior medical records, please call our front office.

BILLING

We would like to thank you for choosing Oak Tree Medical Center. We are contracted with most large PPO companies, Worker's Comp carriers and Medicare. If you have any billing questions, please call our billing company, Oak Tree Healthcare, at (626) 577-3415.

Please read the following policies to ensure timely payment:

Correct Information
Your correct and up to date information in our scheduling system is very important. If you have moved, received new benefits at work or your insurnace card has changed, please let us know. Failure to update your account may lead to denials or increased patient financial responsibility.

Pre-Authorization
Often times insurance companies require pre-authorization for medical procedures and exams. Although we assist patients and referring physician offices in obtainging these authorizations, we are not responsible for ensuring pre-authorization.

Deducitbles/Copays
We collect all applicable deductibles and copays for services performed. Patients should be aware of their medical insurance benefits prior to their arrival, to obtain this information, please call the customer service number on the back of your insurance card.

For patients that have unusally high deductibles or copays, we may require an upfront payment or deposit be made on your account.

Auto Accidents/Work Injurys
Prior to your arrival, we require all patients to disclose the nature of their injury. If your injury is auto or work related we must be informed in order to process proper authorization. Failure to accurately disclose the nature of your injury may result in increased financial responsibility.
